The Greenville County Coroner's Office ruled Tuesday that Hayden Panettiere, 36, died August 16 from an accidental overdose. The cause: toxic effects of fentanyl, 4-ANPP (a fentanyl analog), alprazolam, methocarbamol, and quetiapine. The autopsy found no signs of trauma.

In May, Panettiere appeared on CBS News to promote her memoir This Is Me: A Reckoning, which addressed her long public battle with alcohol and opioid dependence. She told the network she was "better, healthier than I've been in a long time." She checked out of a Malibu rehabilitation facility weeks before her August 16 death.

What investigators found at the scene

Emergency dispatch received the 911 call at 1:51 PM at Judson Mill Lofts in Greenville, where Panettiere had been staying. Her boyfriend Brian Hickerson and his brother Zach found her unconscious. Zach told TMZ that Brian administered Narcan before calling 911. Paramedics applied advanced cardiac life support measures. She was pronounced dead at 2:32 PM.

Blue pressed pills and a straw tested positive for fentanyl on an apartment table. A white powdery substance was found in a sunglasses case in her suitcase. Hickerson showed investigators a bag he described as her medication.

'Better, healthier': the timeline

This Is Me: A Reckoning was published in May 2026 and became a bestseller. In it, Panettiere detailed her struggles with addiction dating back to her teenage years. The book sold out across Greenville on what would have been her 37th birthday, August 21, five days after she died.

She entered a Malibu rehabilitation facility before her death and was released weeks before the August 16 incident, according to the AP's account of the coroner's report. The exact check-in and discharge dates were not included in the report.

The DEA investigation is still open

The Greenville Police Department confirmed its independent investigation "remains active and has not been closed." Detectives "will continue to follow leads and gather information as we work toward resolving this case." The coroner has administratively closed its case with the accidental ruling. Law enforcement has not. The key open question is who supplied the pressed fentanyl pills found at the scene. Panettiere was a Golden Globe-nominated actress who played Claire Bennet in NBC's 'Heroes' and Juliette Barnes in 'Nashville'. She was included in the Primetime Emmy Awards In Memoriam segment. Her representatives and Hickerson's counsel had not commented at press time.
